FirstLantic Healthcare Professional Care Management program specializes in individualized care for the aged, injured or disabled.

We provide each individual client with a Care Manager who uses their knowledge, expertise and skills to make care recommendations. Our devoted and compassionate Care Managers work privately and individually with pediatric and adult clients to conduct a thorough assessment. We then develop and implement a plan of care designed to meet the client’s specific needs. Possible client needs can include:
