A Professional Care Manager is a health care professional who specializes in aging, disability and injury. At FirstLantic Healthcare, our Professional Care Managers are all licensed or certified in their respective fields of clinical practice. We employ:

• Licensed Clinical Social Workers (LCSWs)
• Occupational Therapists Registered(OTRs)
• Registered Nurses (RNs)
• Certified Manager of Care (CMC)

The care manager works privately and individually with each client and their families or loved ones to create a short and long term plan of care that meets the unique needs of each client. This is achieved by completing an extensive and sensitive evaluation of the client in the following areas:

• Medical Situation
• Psychosocial Status
• Current Living Arrangements
• Family Dynamics
• Physical Ability
• Emotional Well-Being
• Financial Viability

Our professional care managers will assist in accessing already existing community resources, as well as locating any other entitlements and services that will best meet the individual needs of our clients.

We continually communicate with out of town family members and medical/healthcare professionals to assure continuity and quality of care.

Do I really need a professional care manager? Before you hire a care manager, determine if you have the time, resources and expertise to handle the problems you are experiencing. Also ask yourself if you feel comfortable or qualified to provide the desired level of supervision and attention to your loved one’s issues. Does insurance coverage professional care management? No, insurance does not cover professional care management services. It is paid for privately, on an hourly basis for time spent working for you.
